WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK AGREEMENT

This agreement is entered into between The Iron Bar and {name} (hereinafter referred to as “Member”), residing at {address}, with a date of birth of {dob} and contact number {phone}.

ASSUMPTION OF RISK

I, {name}, acknowledge that participation in physical fitness activities and use of The Iron Bar’s facilities, equipment, and services involves inherent risks, including but not limited to injury, illness, muscle strain, broken bones, heart attack, or even death. I voluntarily accept and assume all such risks, whether known or unknown, and take full responsibility for my participation.

RELEASE OF LIABILITY

In consideration for being granted access to The Iron Bar, I hereby release, waive, discharge, and hold harmless The Iron Bar, its owners, staff, contractors, affiliates, and any other associated parties from any and all liability, claims, demands, actions, or causes of action related to any loss, damage, or injury that may be sustained while on the premises or while using any equipment or participating in any activity, including any negligence on the part of The Iron Bar.

This release applies to personal injury, property damage, theft, or death, whether caused by negligence or otherwise.

Additionally, I acknowledge that The Iron Bar and the property owner are not responsible for any conduct, services, advice, or interactions with third-party trainers operating within the facility. While The Iron Bar will ensure these trainers operate fairly and safely, any concerns, disputes, or issues must be addressed directly with the trainer involved.

RULES AND CONDUCT

I agree to abide by all posted gym rules and follow the instructions of The Iron Bar staff. I understand that failure to comply may result in suspension or permanent termination of my membership without refund.

I agree to:

• Use all gym equipment responsibly and safely

• Re-rack all weights and return equipment to their proper location after use

• Treat other members and staff with respect

• Maintain proper hygiene and clean equipment after use

• Avoid any behavior that is disruptive, unsafe, or unlawful

I understand that The Iron Bar reserves the right to charge $0.25 per pound for any weights or equipment left out or not properly returned after use. This policy ensures a clean, safe, and respectful environment for all members.

MEDICAL ACKNOWLEDGMENT

I certify that I am in good physical condition and have no known medical conditions that would prevent me from safely participating in gym activities.

I understand it is my responsibility to consult with a physician before starting any new fitness or nutrition program.

PHOTO & VIDEO RELEASE (Optional)

I grant permission to The Iron Bar to use photographs and/or videos of me for promotional and marketing purposes, including social media and website content.
I do not grant permission for media usage.
