WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK AGREEMENT

This agreement is entered into between The Iron Bar and {name} (hereinafter referred to as “Member”), residing at {address}, with a date of birth of {dob} and contact number {phone}.

ASSUMPTION OF RISK

I, {name}, acknowledge that participation in physical fitness activities and use of The Iron Bar’s facilities, equipment, and services involves inherent risks, including but not limited to injury, illness, muscle strain, broken bones, heart attack, or even death. I voluntarily accept and assume all such risks, whether known or unknown, and take full responsibility for my participation.

RELEASE OF LIABILITY

In consideration for being granted access to The Iron Bar, I hereby release, waive, discharge, and hold harmless The Iron Bar, its owners, staff, contractors, affiliates, and any other associated parties from any and all liability, claims, demands, actions, or causes of action related to any loss, damage, or injury that may be sustained while on the premises or while using any equipment or participating in any activity, including any negligence on the part of The Iron Bar.

This release applies to personal injury, property damage, theft, or death, whether caused by negligence or otherwise.

Additionally, I acknowledge that The Iron Bar and the property owner are not responsible for any conduct, services, advice, or interactions with third-party trainers operating within the facility. While The Iron Bar will ensure these trainers operate fairly and safely, any concerns, disputes, or issues must be addressed directly with the trainer involved.

RULES AND CONDUCT

I agree to abide by all posted gym rules and follow the instructions of The Iron Bar staff. I understand that failure to comply may result in suspension or permanent termination of my membership without refund.

I agree to:

• Use all gym equipment responsibly and safely

• Re-rack all weights and return equipment to their proper location after use

• Treat other members and staff with respect

• Maintain proper hygiene and clean equipment after use

• Avoid any behavior that is disruptive, unsafe, or unlawful

I understand that The Iron Bar reserves the right to charge $0.25 per pound for any weights or equipment left out or not properly returned after use. This policy ensures a clean, safe, and respectful environment for all members.

MEDICAL ACKNOWLEDGMENT

I certify that I am in good physical condition and have no known medical conditions that would prevent me from safely participating in gym activities.

I understand it is my responsibility to consult with a physician before starting any new fitness or nutrition program.

PHOTO & VIDEO RELEASE (Optional)

I grant permission to The Iron Bar to use photographs and/or videos of me for promotional and marketing purposes, including social media and website content.
I do not grant permission for media usage.

REMINDER TO ALL MEMBERS

Keeping The Iron Bar clean and organized is everyone's responsibility. We take pride in our gym, and we expect every member to do the same.

Please remember to:

• Re-rack every weight where you got it from.

• Clean up any chalk you use, including scrubbing chalk off the bars.

• Return all cable attachments and equipment to their designated locations.

• If you spill a drink or make a mess, clean it up.

• Wipe down all gym equipment after use. Sweat and body oils damage upholstery and cause premature cracking.

Re-Rack Policy

Failure to re-rack your weights will result in the following:

1st Offense:
Up to 100 push-ups or 100 reps using the machine and weight you left set up.

2nd Offense:
Up to 200 reps using the machine and weight you left set up.

All penalty sets must be recorded and submitted. If they are not recorded, they will be considered incomplete, and your account will be charged accordingly.

3rd Offense:
An automatic charge of $0.25 per pound of any weight left out will be applied directly to your account. No exceptions.

These rules are not meant to be difficult, they exist to maintain a clean, organized gym that everyone enjoys training in. Respect the equipment, respect the gym, and respect your fellow members.

If these expectations are not something you're comfortable with, we completely understand and will gladly assist you in canceling your membership so you can join a corporate gym where these standards may not be enforced.
